Qualifications

• Bilingual Spanish required.

Responsibilities

• HRSN Screening & Eligibility Assessment: conduct outreach, perform screenings, assess eligibility for enhanced HRSN services.
• Navigation & Closed-Loop Referrals: connect members to resources, track referrals, and ensure needs are addressed.
• Documentation & Community Partnership: document activities, maintain records, and engage community partners.
• Nutrition Focus: Education & Access: connect members to healthy food resources and provide nutrition education.
