Zusammenfassung

The Chinese-American Planning Council (CPC) is seeking a full-time Nutrition Navigator for the Multi-Social Services Center in Lower Manhattan. You will prescreen eligibility for SNAP and related benefits and assist applicants through the AccessNYC.gov process, while advocating for enrollment and coordinating with HRA and partners.

Responsibilities include outreach across boroughs, community education, and reporting to funders.

Qualifikationen

• Bachelor’s degree preferred; high school with social service experience considered.
• Experience with SNAP and government benefits is a plus.
• Bilingual in Chinese (Cantonese and/or Mandarin) and English required.

Aufgaben

• Review eligibility for SNAP and other benefits for individuals and families.
• Assist with SNAP applications and follow-up to ensure benefits are received.
• Engage with HRA and partners to resolve SNAP issues and advocate for enrollment.
• Process SNAP applications via AccessNYC.gov.
• Respond to information requests by phone and follow up with callers.
• Pre-screen for other entitlement benefits for referrals.
• Public speaking and outreach at community events and sites.
• Serve as liaison between community members and city departments.
• Prepare reports for funders and attend required meetings.
