Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking a Community-based Clinician to deliver primary and urgent care in patients’ homes, focusing on care transitions after hospital discharge. You will work with the Core Clinical Team to coordinate care and optimize outcomes for vulnerable homebound members.

The role requires an Advanced Practice Provider with acute care experience, ACLS certification, and strong EMR skills.

Qualifications

• APN/PA with acute care experience and adult medicine focus.
• ACLS certification required and clinical leadership experience preferred.
• Experience using EMR systems and data reports for patient care.

Responsibilities

• Provide community-based medical and care-coordination services for recent hospital discharges.
• Manage a small panel of homebound members requiring primary care.
• Coordinate care with PCPs, nurses, therapists and community resources to prevent readmissions.
