Job summary

Housing Options & Planning Enterprises, Inc. (H.O.P.E.) is seeking a Community Navigator to connect underserved populations with housing services and wrap-around supports in Prince George's County.

You will perform outreach, assess client needs, develop personalized action plans, and assist with financial service applications while maintaining client confidentiality. Preferred qualifications include an Associate Degree in Human Services, 2–3 years in social services, strong communication skills,

Qualifications

• Associate degree in Human Services or related field; life and employment experience considered in lieu of education.
• Two to three years of experience in social services.
• Knowledge of local community resources and partners.
• Professional, organized, motivated to learn, with strong communication and listening skills.
• Strong time-management skills.
• Proficient with Microsoft Office.
• Ability to work on teams and collaborate.

Responsibilities

• Develop a written client listing of community referrals and resources (food banks, childcare, financial assistance, shelters, etc.).
• Receive and acknowledge referrals from housing counselors, partners, and clients.
• Conduct and document day-to-day outreach to community resources.
• Assess client needs using assessment tools.
• Develop written personalized activities for each client to meet housing goals.
• Assist clients with completing and submitting financial service applications and review for completeness.
• Refer clients to educational, community, and cultural activities in Prince George's County.
• Follow up via phone or email to determine resource connections in the client plan.
• Perform ongoing case management to provide service and problem-solving.
• Represent and market agency at events; may require evenings and weekends.
• Prepare and submit timely information to meet agency reporting requirements.
• Maintain compliance with program funder guidelines.
• Record and update client logs/notes in the Client Management System; maintain confidentiality.
• Participate in weekly staff meetings and training; coordinate with counseling staff.
