Job summary

AbilityFirst in Long Beach seeks a Community Navigator to provide person-centered planning, case management, intake coordination, and community engagement services. You will collaborate with staff, regional center coordinators, and families to tailor services to each participant's goals.

The role requires LIC Director Qualified, strong documentation skills, and regular community outreach. Position is full-time, with a 9–6 schedule and occasional evenings/weekends in Long Beach, CA.

Qualifications

• High school diploma or equivalent.
• Bachelor's in psychology/sociology/marriage & family or 12 ECE units or 4+ years related experience preferred.
• LIC Director Qualified under Title 17/22 required.
• 2+ years experience in developmental disability services.
• Strong written and verbal communication skills.
• MS Office and data management skills required.
• Valid driver's license and willingness to work evenings/weekends.
• Background checks including Live Scan completed.

Responsibilities

• Attend required 2-day Person-Centered Thinking training.
• Conduct intake meetings and screenings as first level of intake.
• Develop PCPs and 1-page descriptions; document progress.
• Collaborate with Regional Center coordinators and families.
• Meet with participants monthly to review goals and placement.
