Advantage Behavioral Health LLC. is seeking a Case Manager to assess client needs, connect them with community resources, plan services, and coordinate care across teams in our California program.

You will handle aftercare planning, crisis intervention, and third-party requests, with involvement in treatment team discussions and ongoing coordination of care. This role emphasizes teamwork, thorough documentation, and a compassionate, professional approach to improve outcomes.

Qualifikationen

• Experience in case management and connecting clients to community resources.
• Strong written and verbal communication skills.
• Ability to coordinate with Admissions, Clinical and other teams.

Aufgaben

• Coordinate daily case management needs assisting clients with securing resources.
• Participate in weekly Treatment Team Meetings.
• Discharge & Aftercare planning.
• Facilitate groups at least once per week.
• Maintain weekly communication with client’s social supports.
• On-Call/After-Hours Phone coverage, as needed.
• Collaborate with Admissions and Clinical to assist in refer out process.
• Liaise with BHT and Clinical staff to ensure best client outcomes.
