Job summary

Lao Family Community Development, Inc. in San Pablo, CA is seeking a full-time Case Manager to support WIOA participants and CalAIM initiatives. You will coordinate with AJCC and partner organizations to link clients to employment, health, and social services.

You will assess needs, plan individualized services, provide counseling and job readiness support, and help participants secure training and placement. Strong coordination, documentation, and culturally competent care are essential.

Qualifications

• Bachelor's degree in a related field or 1 year of relevant experience in case management or workforce development may be considered in lieu of a degree.
• Bilingual proficiency in Spanish and English is highly desirable.

Responsibilities

• Conduct comprehensive participant assessments to develop individualized care management and employment plans.
• Perform community outreach, resource fairs, and referrals to workforce programs and services.
• Facilitate workshops, counseling sessions, and job readiness activities.
• Assist participants with resumes, cover letters, and Individual Employment Plans (IEPs).
• Provide intensive case management including intake, eligibility, enrollment, and follow-up services for up to 12 months.
