Job summary

Carolina Outreach, LLC is seeking a full-time Community Support Team (CST) Professional Team Lead to provide community-based therapy and case management to adults with serious mental health challenges in North Carolina. You will conduct assessments, provide individual and family therapy, and guide service delivery with person-centered plans.

Role includes supervising staff where applicable, coordinating care with ACT services, and ensuring compliant documentation.

Qualifications

• Master’s degree from an accredited university in Counseling or a related field.
• Valid licensure to provide mental/behavioral healthcare in North Carolina (LCSW/LCSWA/LCMHC/LCMHCA/LMFT or Licensed Psychologist).
• Minimum two years of experience with mental/behavioral health services and ability to write person-centered plans.

Responsibilities

• Provide individual, family, and/or group therapy sessions.
• Develop and implement person-centered plans and monitor client progress.
• Coordinate care with ACT and other services when appropriate.
• Maintain accurate clinical documentation and progress notes.
• Supervise clinical staff as applicable and participate in care-team collaboration.
• Respond to crises promptly and ensure service continuity.
