Job summary

Qualifications

• Bachelor's degree in a human service field such as psychology, social work, education or rehabilitation.
• At least 2 years of experience in a behavioral health or human service setting.

Responsibilities

• Conduct screenings and assessments within person-centered treatment planning.
• Identify strategies or options to improve daily living and community integration.
• Provide individual and group counseling to restore stability and independence.
• Develop goals with clients and families, using strengths and resources.
• Support coping skills and symptom management aligned with treatment plans.
• Coordinate care with other providers and document accurately in health records.
