Brown University Health seeks a dedicated professional to provide case management, rehabilitation, and crisis intervention for adults with serious mental illness in home, clinic, and community settings. • Associate’s degree in human services or related field is required.
• Bachelor’s degree in related field is preferred.
• Bilingual fluency in Spanish/English or other critical language is preferred.

Responsabilités

• Provide case management, rehabilitation, crisis interventions, and outreach to adults with serious mental illness.
• Engage clients in assessments, diagnosis, intervention, and treatment with an interdisciplinary team.
• Document all services in the electronic medical record following established protocols.
• Coordinate treatment plans and discharge planning with other providers.
• Assist clients with insurance, medical care, housing, employment, and social supports.
• Provide psychoeducation to clients and families to improve adherence and functioning.
