Job summary

Qualifications

• Associate’s degree in human service field required; Bachelor’s degree preferred.
• Experience or interest working with individuals living with mental illness, substance use disorders, and/or psychosocial stress.
• Bilingual fluency in Spanish/English or other critical language preferred.

Responsibilities

• Provides clinical interventions such as needs assessment, resource navigation, skill teaching, and care coordination to adult clients in community settings.
• Engages clients to participate in treatment services and documents progress.
• Delivers services primarily in community settings, including homes, businesses, or outdoor spaces.
• Transports clients in a personal vehicle when clinically appropriate.
• Maintains detailed, timely EMR documentation following protocols.
• Assesses client functioning and updates treatment plans accordingly.
• Assists clients with obtaining insurance, medical care, employment, housing, and social supports.
