Job summary

Milwaukee Center for Independence Inc. in Milwaukee, WI is seeking a Registered Nurse to support community-based clients with mental health needs.

The RN will administer medications, coordinate care with the treatment team, and monitor medical conditions to promote wellness within DHS guidelines. The role requires collaboration with hospitals, home/community settings, and program staff, with responsibilities including documentation, care planning, and patient education.

Qualifications

• Associate degree in Nursing required.
• Minimum 1,000 hours of clinical experience with adults with severe and persistent mental illness.
• Current CPR certification or training to obtain within 90 days; Wisconsin driver's license.

Responsibilities

• Understand and apply DHS 63 treatment guidelines in practice.
• Provide medications administration and coordination in community and home settings.
• Maintain medication records and collaborate with MD and pharmacy on changes.
• Discharge planning communication with hospitals and long-term care facilities.
• Provide comprehensive assessments, care planning, and nursing interventions within program guidelines.
• Document services thoroughly and timely to maintain client records.
• Provide training to staff on health-related issues and supervise interns when appropriate.
