Job summary

Centers for Independence in Milwaukee is seeking a registered nurse to support individuals with mental illness within an integrated treatment team, providing medical support and medication management.

The RN will coordinate care, perform assessments, and document thoroughly while guiding discharge planning and collaboration with physicians. Travel up to 25% and EHR use are expected; requires a 2-year nursing degree and Wisconsin license.

Qualifications

• Associate’s degree in Nursing is required.
• At least 1,000 hours of clinical experience with adults with severe mental illness.
• Current CPR certification or obtained within first 90 days of employment.
• Valid Wisconsin driver's license and auto insurance.
• Knowledge of psychopharmacology and community health resources.
• Proficiency with Microsoft Word, Outlook, Teams, and Excel.
• Excellent communication in English, both written and spoken.

Responsibilities

• Understand and follow DHS 63 treatment guidelines.
• Provide support and administer medications to clients in the community or home.
• Assist clients in medication management and maintain medication records.
• Coordinate discharge planning with hospitals or psychiatric providers.
• Provide comprehensive assessment, care planning, and nursing interventions.
• Document services and maintain thorough client records.
• Offer health education and staff training as appropriate.
• Provide transportation to clients as needed by the team.
• Perform additional duties as assigned within the ACT team.
