Job summary

The Elizabeth Layton Center (ELC) seeks an RN to join its Assertive Community Treatment (ACT) team in Ottawa, KS. The RN will provide nursing services within a comprehensive community-based program, collaborating closely with physicians and a multidisciplinary team to support adults with serious mental illness.

Key duties include medication management, health education, and care coordination, with a focus on reducing ER visits and hospitalizations.

Qualifications

• Must be a registered nurse with Kansas licensure.
• Experience in community-based ACT settings is preferred.
• Strong communication and collaboration skills are required.
• Ability to work in a multidisciplinary team and respond to crises.
• Must pass background checks and have a valid driver's license.

Responsibilities

• Provide nursing assessments and care as part of the ACT team.
• Set up, monitor, administer medications and injections.
• Coordinate with physicians, hospitals, pharmacies and other agencies.
• Provide health education and wellness coaching to clients.
• Maintain clinical documentation per agency guidelines.
• Participate in staff meetings and meet performance expectations.
• Maintain fidelity to the ACT model of care.
