Community Counseling Center Inc seeks a qualified professional to deliver community support services for clients enrolled in the CPRC program. You will coordinate mental health service delivery, assess client adjustment, and link with community resources to reduce hospitalization risk.

Join a multidisciplinary team delivering comprehensive assessment and treatment for clients with chronic psychiatric disorders. Valid MO driver’s license and mileage travel required, plus evenings may be needed.

Qualifications

• Qualifying experience must include delivery of service to individuals with mental illness, substance abuse disorders, or developmental disabilities.

Responsabilités

• Provide community support services to children and adult clients enrolled in CPRC program.
• Maintain a caseload as assigned including in the community and during hospitalizations.
• Document all interactions with clients in adherence to agency guidelines and complete all documentation prior to the agency specified deadline.
• Maintain direct services productivity in accordance with assigned caseloads.
• Provide training/education to clients in daily living skills.
• Maintain positive attitude toward agency and clientele.
• Monitor completion of treatment plan goals and objectives and maintain contact with treatment team regarding progress and any concerns.
• Refer to appropriate resources within the community to assist clients in maintaining/improving their functionality and to develop a personal support system.
• Responsible for transporting clients in agency and/or personal vehicle as deemed necessary and appropriate to meet program requirements.
• Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.

Bachelor's degree or higher (or four-year combination)
