Compass Health Network is seeking a Community Support Specialist for the Adult CPRC in Wentzville. You will work with adults to support behavioral health and recovery goals, meeting clients in homes, offices and in the community. Regular driving and client transportation are required.

Ideal candidates have a related degree or equivalent experience, strong empathy, and excellent time management and writing skills. This role emphasizes hands-on, relationship-based care within a collaborative team.

Qualifikationen

• High School/GED required
• Associate’s or Bachelor’s degree in a human service-related field preferred
• Qualifying experience includes delivering services to adults with behavioral health needs.

Aufgaben

• Support clients in reaching recovery and wellness goals.
• Provide coaching and education on life skills and community resources.
• Help clients access housing, healthcare, employment and other essential services.
• Work closely with healthcare providers, families, and community partners.
• Assist with appointments and provide transportation as clinically needed.
• Maintain accurate, timely documentation.

Ausbildung
