Job summary

CMSNE is seeking an entry-level Case Manager to join its community-based psychosocial rehabilitation services. The role involves planning, coordinating, providing and monitoring therapeutic, rehabilitative, and community integration services for adults with severe psychiatric disability to promote independent community living.

The ideal candidate will have a relevant degree and a driver’s license, with the ability to transport clients as needed and demonstrate sound clinical judgment and time

Qualifications

• Bachelor’s or higher in psychology, social work, psychosocial rehabilitation or related human services field.
• Understanding of mental illness and desire to reduce stigma and effectively support this population.
• Valid driver’s license and ability to transport clients as needed.

Responsibilities

• The Case Manager plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services for adults with severe psychiatric disability designed to help them live as independently as possible in the community.
