Job summary

Monadnock Family Services in New Hampshire seeks an entry-level Case Manager to provide community-based psychosocial rehabilitation for adults with serious mental illness. You will assess, plan, coordinate and monitor services to support clients' independence and community integration.

You will work with a multidisciplinary team, apply good clinical judgment, time management and empathetic communication to deliver supportive, evidence-based care in the community setting.

Qualifications

• Bachelor’s degree in psychology, social work, psychosocial rehabilitation or related field.
• Or Associate’s degree with two years’ experience working with individuals with mental illness.
• Understanding of mental illness and desire to reduce stigma and support this population.
• Valid driver’s license and ability to transport clients as needed.

Responsibilities

• Plan, coordinate, provide and monitor therapeutic, rehabilitative and community integration services for adults with severe psychiatric disability.
• Demonstrate good clinical judgment, time management and appropriate treatment in the community.
• Support clients to integrate as independently and fully into the life of the community as possible.
