Job summary

Monadnock Family Services in New Hampshire seeks an entry-level Case Manager to provide community-based psychosocial rehabilitation for adults with serious mental illness. You will assess, plan, coordinate and monitor services to support clients' independence and community integration.

You will work with a multidisciplinary team, apply good clinical judgment, time management and empathetic communication to deliver supportive, evidence-based care in the community setting.

Qualifications

• Bachelor’s degree in psychology, social work, psychosocial rehabilitation or related field.
• Or Associate’s degree with two years’ experience working with individuals with mental illness.
• Understanding of mental illness and desire to reduce stigma and support this population.
• Valid driver’s license and ability to transport clients as needed.

Description

This is the entry level professional position in community based psychosocial rehabilitation services for individuals with serious and persistent mental illness. The Case Manager plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services for adults with severe psychiatric disability designed to assist these individuals to integrate as independently and fully into the life of the community as possible. Case Manager works independently and must demonstrate good clinical judgment, decision making, time management and appropriate treatment for consumers in the community.

Requirements
