Qualifications

Responsibilities

• Coordinate mental health services across providers and agencies.
• Establish measurable goals based on client needs.
• Track client progress and adjust services as necessary.

Skills

Mental Health Professional (MHP) or Qualified Mental Health Professional (QMHP)

Experience in community-based case management or care coordination

Knowledge of community resources in the Chicagoland area

Experience working with a homeless or high-risk of homelessness population

Experience with individuals with serious mental illness (SMI)

Education
