Job summary

PWP Health in Chicago, IL seeks a Caseworker to coordinate comprehensive behavioral health services across providers, agencies, and community systems. The role is within a community mental health model, meeting people where they are and offering flexible scheduling.

You will partner with schools, housing programs, and support groups to help clients progress toward independence, with emphasis on recovery, access to resources, and integrated care in everyday settings.

Qualifications

• Must meet Illinois standards as Mental Health Professional (MHP) or QMHP.
• Bachelor's or Master's in Social Work, Psychology, Counseling, or related behavioral health field.
• Experience in community-based case management or care coordination (preferred).
• Knowledge of community resources in the Chicagoland area.
• Experience with homeless or high-risk homelessness populations.
• Experience with serious mental illness (SMI) or co-occurring disorders.

Responsibilities

• Coordinate mental health and supportive services across providers, social service agencies, schools, housing programs, and other community partners.
• Establish measurable goals aligned with client strengths, needs, and recovery objectives.
• Track client progress and adjust services as needed.
• Ensure all services are documented to support clinical necessity and billing compliance.
• Support clients in navigating complex systems and achieving greater independence and stability.
