You will partner with schools, housing programs, and support groups to help clients progress toward independence, with emphasis on recovery, access to resources, and integrated care in everyday settings.

Qualifikationen

• Must meet Illinois standards as Mental Health Professional (MHP) or QMHP.
• Bachelor's or Master's in Social Work, Psychology, Counseling, or related behavioral health field.
• Experience in community-based case management or care coordination (preferred).
• Knowledge of community resources in the Chicagoland area.
• Experience with homeless or high-risk homelessness populations.
• Experience with serious mental illness (SMI) or co-occurring disorders.

Who You Are

• You Must meet Illinois standards as a Mental Health Professional (MHP) or Qualified Mental Health Professional (QMHP) .
• You have a Bachelor's or Master's degree in Social Work, Psychology, Counseling, or a related behavioral health field.
• Your have experience in community-based case management or care coordination, preferred
• You have knowledge of community resources in the Chicagoland area
• You have experience working with a homeless or high-risk of homelessness population.
• You have experience working with individuals with serious mental illness (SMI), co-occurring disorders, or high-acuity behavioral health needs.

The anticipated pay for this role is $24 per hour. Salary offers are made based on the candidate’s qualifications, experience, skills, and education as they directly relate to the requirements of the position, budget for the position and cost of labor in the market in which the candidate will be hired.
