Qualifications

• High School diploma or GED preferred.
• Knowledge of I/DD and mental health a plus.
• Must be at least 18 and have a valid driver's license.

Responsibilities

• Provide support to individuals as identified in IPOS.
• Assist with medication setup and/or delivery per policy.
• Assist in finding and maintaining volunteer opportunities.
• Collaborate with providers, families and case managers on assessments and planning.
• Preserve health and safety to support independence.
• Prepare progress notes and required documentation.
