Job summary

Responsibilities

• Provides necessary supports as identified by the Person-Centered Planning Team and coordinates with families and program partners.
• Teaches and supports daily living tasks such as cleaning, meal planning, budgeting, and shopping.
• Assists individuals to develop skills for independent living following the Person-Centered Plan.
• Monitors medication as prescribed and assists with administration as needed.
• Engages in crisis intervention when required and provides first aid or CPR.
• Plans and participates in social activities with individuals and other staff.
• Documents activities and behaviors in the electronic records daily and maintains program compliance.
• Transports individuals and provides mobility training to attend appointments and community activities.
• Ensures safety, sanitation, and security in the home and reports maintenance needs.
• Follows policy and procedures and works cooperatively with the support team.
