Job summary

AHRC Nassau in New York is seeking Direct Support staff to provide ongoing guidance and support to individuals in the Community Living Services program. You will assist with self-care, health, safety, meals, transportation, and daily routines to promote independence and quality of life.

Responsibilities include implementing treatment plans, documenting medical notes, mentoring new staff, and coordinating with the inter-disciplinary team. A valid NYS driver’s license is required.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license and maintain qualified driver status.
• Ability to communicate and understand written and oral directives.

Responsibilities

• Provides ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life and wellbeing.
• Supports individuals in community participation and maintains natural supports and relationships.
• Ensures safe, clean homes by assisting with and participating in household chores.
• Follows treatment plans and uses adaptive and safety equipment to promote independence.
• Assists with meal preparation, diet adherence, portion control, and cooking skills.
• Assists individuals with medication administration according to AMAP policy.
• Implements and documents treatment plans, residential goals, and medical notes to aid independence and safety.
• Adheres to overtime, scheduling, and staffing policies to ensure daily quality support.
• Participates in interdisciplinary team meetings to discuss progress and goals.
• Provides transportation for medical appointments, shopping, and outings.
• Mentors new Direct Support Staff by demonstrating routines and reviewing training materials.
• Participates in house meetings and maintains certification and training expectations.
• Performs other duties as requested by supervisor.
