Job summary

Toolworks seeks compassionate Direct Support Professionals for Case Lead roles in ILS and SLS in the San Francisco Bay Area. Full-time or part-time schedules available; compensation is $29.00 per hour.

You will provide in-home instruction, case management, and daily living skills training to adults with disabilities, helping them live as independently as possible and participate in their communities. Strong communication and adaptability are essential, with background checks and vaccinations

Qualifications

• Minimum 2 years' experience as a DSP or related role supporting adults with disabilities.
• B.A. in Special Education, Rehabilitation Counseling, or related field preferred.
• ASL/Spanish/Cantonese language skills are highly desirable.

Responsibilities

• Develop individualized service plans and support goals for clients in Supported Living.
• Assist clients in accessing benefits and building natural supports.
• Foster self-sufficiency and community participation through instruction and support.
• Provide case notes, incident reports, and monthly billing documentation.
