Job summary

Qualifications

High School Diploma or equivalent required.
Valid Driver’s License.
Ability to obtain CPR, First Aid, and Mandt Certifications.
Ability to read, write and speak English.
Basic computer skills (Typing, Microsoft Word).

Responsibilities

Facilitate and assist consumers in achieving wellness and daily living goals.
Documentation as required by Valley HealthCare System, Medicaid, OSHA, and other licensing agencies.
Complete EMR documentation during each daily episode of care.
Attend Interdisciplinary Team meetings and participate in Treatment Plan development.
Ensure safety, health and well-being of consumers on or off site.
Implement treatment/behavior plans in compliance with Valley HealthCare System’s and State guidelines.
Complete program-specific documentation in a timely and accurate manner.
Transportation of consumers to appointments and activities using a company vehicle or employee’s personal vehicle.
Support and help in the training of new employees.
Attends in-service trainings, mandated meetings and completes Essential Learning trainings as assigned.
Engage consumers in achieving their daily goals and objectives through active treatment.
