Job summary

Southeastern Directions for Life is seeking a full-time Community Life Facilitator at our Education & Integration Adult Day Program location to support adults with developmental disabilities by teaching self-care, social, vocational, independent living, and communication skills.

You will work closely with families and attend team meetings, handling program writing, assessments, data, monthly reports, I-Pops, and PCT tools.

Qualifications

• High school diploma or equivalent; Bachelor's degree in either human services or education preferred.
• Must have a current driver's license, car insurance, and a driving record acceptable to Southern’s insurance provider.
• Must pass a comprehensive criminal background check to include fingerprinting, Dept. of Social Services Central Registry check, Motor Vehicle background check, and drug screening.

Responsibilities

• Teach adults with Developmental Disabilities self-care, social, vocational, independent living, and communication skills.
• Communicate with family members to maintain positive relationships.
• Advocate for the people we support; attend team meetings and complete program writing, assessments, data, monthly reports, I-Pops, and PCT tools.
• Ensure community interactions are safe, clean, and conducive to learning; support operations 24/7, including holidays when necessary.

JOB SUMMARY:

This position provides services to adults with Developmental Disabilities. You will work directly with the people we support teaching them self-care, social, vocational, independent living, and communication skills. Communicate with family members of the people we support in an effort to maintain a positive relationship between them and other family members. May act as an advocate for the people we support in communicating their needs, self-expression, and goals. Provide community interaction that is safe, clean, and conducive to learning. The CLF is responsible for attending team meetings, program writing, assessments, data, monthly reports, I-Pops, and PCT tools. Southern operates 24/7, 365 days a year. Flexibility during emergencies and inclement weather is required. Must be able to work holidays when necessary.
