Volunteers of America–Greater New York (VOA–Greater New York) invites applications for a Community Liaison Wellness Coordinator in Bronx, NY. Qualifications

• Bachelor’s degree or 60 college credits plus two years relevant experience developing community linkages.
• Demonstrated verbal and written communication ability, including basic computer skills.

Responsibilities

• Manage a small caseload.
• Manage the resident intake and assessment process to orient the resident to the program and produce a comprehensive assessment of resident service needs.
• Develop with the resident a mutually agreed-upon individual service plan reflecting assessment findings and goals; update as required.
• Engage outside providers to develop quality linkages to support resident needs.
• Develop with the resident a mutually agreed-upon wellness plan and update as necessary.
• Meet regularly with residents to assist them in obtaining needed services/ benefits and provide supportive counseling.
• Document counseling and linkage services per guidelines; collect data and participate in evaluation and QA activities.
• Design, conduct and coordinate staff trainings and resident presentations and groups.

Position: Community Liaison Wellness Coordinator

The Community Liaison Wellness Coordinator is a key provider of services and will be responsible for identifying and developing strategic linkages with community-based organizations to enhance onsite services provided to program residents. Community Liaison/Wellness Coordinator will focus on gauging three key areas of importance to the wellbeing of every tenant: general wellness; financial stability; and isolation/depression.

Community Liaison Wellness Coordinator Principal Responsibilities

• Design, conduct and coordinate staff trainings and presentations
• Provide opportunities for corporations and community groups to support program services. Design, conduct and coordinate resident presentations and groups
