Volunteers of America–Greater New York (VOA–Greater New York) invites applications for a Community Liaison Wellness Coordinator in Bronx, NY. You will identify and develop linkages with community organizations to enhance onsite services for program residents, focusing on general wellness, financial stability, and isolation/depression.

Qualifications

• Bachelor’s degree or 60 college credits plus two years relevant experience developing community linkages.
• Demonstrated verbal and written communication ability, including basic computer skills.

Responsibilities

• Manage a small caseload.
• Manage the resident intake and assessment process to orient the resident to the program and produce a comprehensive assessment of resident service needs.
• Develop with the resident a mutually agreed-upon individual service plan reflecting assessment findings and goals; update as required.
• Engage outside providers to develop quality linkages to support resident needs.
• Develop with the resident a mutually agreed-upon wellness plan and update as necessary.
• Meet regularly with residents to assist them in obtaining needed services/ benefits and provide supportive counseling.
• Document counseling and linkage services per guidelines; collect data and participate in evaluation and QA activities.
• Design, conduct and coordinate staff trainings and resident presentations and groups.
