Job summary

Enhabit Home Health & Hospice seeks a dynamic relationship manager to bridge Enhabit with physicians, hospitals, senior living communities and other referral sources. You will educate partners about Enhabit services, support post-acute care transitions, and drive referral growth to improve patient outcomes and satisfaction.

Required: Bachelor's degree in healthcare administration, marketing, or related field (or equivalent experience); two years in healthcare business development or sales; valid

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field or equivalent experience.
• Two years of experience in healthcare business development, community relations, marketing, or sales.
• Understanding of Medicare and home health regulations.
• Valid driver's license and automobile insurance.
• Strong interpersonal, communication and presentation skills.
• In-office attendance is essential (except remote roles).

Responsibilities

• Serve as the primary relationship manager between Enhabit and referral sources (physicians, hospitals, senior living communities, associations, etc.).
• Educate referral sources about Enhabit services and program requirements.
• Support referral growth and navigation of post-acute care transitions to improve patient outcomes and satisfaction.
