Job summary

Enhabit Home Health & Hospice is seeking a proactive professional to serve as a primary liaison between Enhabit and referral sources such as physicians, hospitals, and senior living communities. You will educate partners on services and program requirements to support post-acute care transitions and drive referral growth.

Ideal candidates bring a bachelor’s degree in healthcare administration or related field, strong interpersonal and presentation skills, and experience in healthcare business

Qualifications

• Requires bachelor’s degree in healthcare administration, marketing, or related field or equivalent experience in nursing, therapy, or social work.
• Strong interpersonal, communication and presentation skills are required.
• Experience in healthcare business development, community relations, marketing, or sales is preferred.
• Knowledge of federal, state, and local laws governing Medicare certified home health and hospice operations is required.
• Valid driver license and automobile liability insurance are required.
• Experience with electronic referral systems or Homecare Homebase (HCHB) is a plus.

Responsibilities

• Serve as the primary relationship manager between Enhabit and referral sources including physicians, hospitals, health systems and senior living communities.
• Support referral growth by educating referral sources on Enhabit services and program requirements to navigate post-acute care transitions.
• Aim to improve patient outcomes, support care transition, and promote satisfaction for patients and referral sources.
