Qualifications

• Bachelor’s degree in social services or related field preferred.
• Certified Addiction Peer Recovery Coach or Certified Peer Support Professional preferred.
• Minimum of two years experience working with addiction/recovery preferred.
• Valid driver's license and car for full-time business use.
• Experience in screening, assessments, treatment and recovery strategies.

Responsibilities

• Engage individuals and families in the therapeutic process and complete assessments.
• Provide home and community-based case management and care coordination.
• Develop case plans and maintain timely documentation for funding.
• Coordinate with community agencies and supports such as housing and health services.
• Respond to client crises during evenings and weekends as appropriate.
• Maintain data in prescribed systems and meet program benchmarks.
