Job summary

Trinity Health PACE, LIFE at St Francis College, Newark, DE, seeks a Community Liaison to grow awareness and referrals for the PACE program. You will build relationships with hospitals, physicians, and community organizations to connect eligible seniors with care and support services.

You will educate referral sources, families, and partners on program benefits, generate qualified referrals, and coordinate with enrollment, clinical, and operations teams to ensure smooth transitions.

Qualifications

• Bachelor’s degree with 2+ years of healthcare sales, outreach, business development, or community engagement.
• Experience in senior care, home care, hospice, PACE.
• Strong networking and relationship-building skills.
• Excellent communication and presentation skills.
• Self-motivated with ability to manage a territory independently.
• Comfortable in assisting seniors with the process in their homes.
• Valid driver’s license and reliable transportation.

Responsibilities

• Initiate, build relationships with and maintain contact with community referral sources to promote the PACE program.
• Develop and maintain referral relationships with hospitals, physicians, rehab centers, senior centers, and community organizations
• Educate referral sources, families, and community partners on program benefits

• Conduct presentations, community events, and outreach activities
• Maintain territory plans and referral pipelines

Position Purpose:

We are seeking a motivated, relationship-driven Community Liaison to grow awareness and referrals for our PACE program. This role is ideal for someone with healthcare sales, senior services, outreach, and provider relations experience.
