Job summary

LIFE at St Francis College, a part of Trinity Health PACE, invites a Community Liaison to expand awareness and referrals for the PACE program in Newark, DE. You’ll cultivate partnerships with hospitals, physicians, and community groups to connect eligible seniors with in-home support and care services.

The role emphasizes relationship-building, community education, and collaboration with enrollment, clinical, and operations teams to ensure smooth transitions for participants.

Qualifications

• Bachelor’s degree with 2+ years in healthcare sales, outreach, business development, or community engagement
• Experience in senior care, home care, hospice, PACE
• Strong networking and relationship-building skills
• Excellent communication and presentation skills
• Self-motivated with ability to manage a territory independently
• Comfortable in assisting seniors with the process in their homes
• Valid driver’s license and reliable transportation

Responsibilities

• Initiate, build relationships with and maintain contact with community referral sources to promote the PACE program
• Develop and maintain referral relationships with hospitals, physicians, rehab centers, senior centers, and community organizations
• Educate referral sources, families, and community partners on program benefits
• Generate qualified referrals and support enrollment growth
• Conduct presentations, community events, and outreach activities
• Maintain territory plans and referral pipelines
• Collaborate with enrollment, clinical, and operations teams to ensure smooth participant transitions

Position Purpose:

We are seeking a motivated, relationship-driven Community Liaison to grow awareness and referrals for our PACE program. This role is ideal for someone with healthcare sales, senior services, outreach, and provider relations experience.
