Trinity Health in Norristown, PA is seeking a Senior Care & Community Outreach Liaison to grow awareness and referrals for our PACE program. This on-site role focuses on building partnerships and educating clinicians and families.

Day-to-day you'll develop referral networks, conduct presentations, and manage territory pipelines, collaborating with enrollment, clinical, and operations teams to support seamless transitions for seniors.

Qualifications

• 2+ years of experience in healthcare sales, outreach, business development, or community engagement.
• Experience in senior care, home care, hospice, PACE.
• Strong networking and relationship-building skills.
• Excellent communication and presentation skills.

We are seeking a motivated, relationship-driven Community Liaison to grow awareness and referrals for our PACE program. This role is ideal for someone with healthcare sales, senior services, outreach, and provider relations experience.

In this position, you’ll build referral partnerships, educate the community, and connect eligible seniors with life-changing care and support services.
