Job summary

St. Joseph Hospice in Houston, TX is seeking a Community Liaison to educate physicians, social workers, and families about hospice services and to build referral networks. This patient-focused role offers ESOP benefits and opportunities to engage with the community.

You will conduct in-services, represent the facility at events, and collaborate with discharge planners and insurance partners to streamline admissions. A degree is preferred, and comfort with Microsoft Office is expected.

Qualifications

• Associate or Bachelor's degree preferred.
• Working knowledge of computers and software packages such as Microsoft Office.
• Great written and verbal communication skills.
• Ability to work with physicians, social workers, case managers, patients and families.

Responsibilities

• Provides education about Hospice services to physicians, social workers, discharge planners, insurance companies, case managers and potential patients/families.
• Assists with referrals and admission process.
• Conducts in-services and direct face-to-face sales calls, as well as represents the facility in community-related activities and marketing events.
