Job summary

St. Joseph Hospice in Houston, TX is seeking a Community Liaison to educate physicians, social workers, discharge planners and other partners about hospice services and referral processes.

The role involves supporting admissions, conducting in-services, attending community events and representing the facility in marketing activities, with competitive pay and benefits including ESOP stock ownership.

Qualifications

• Associate or Bachelor's degree preferred.
• Strong written and verbal communication skills.
• Ability to work with physicians, social workers, case managers, patients and families.

Responsibilities

• Provides education about Hospice services to physicians, social workers, discharge planners, insurance companies, case managers and potential patients/families
• Assists with referrals and admission process.
• Conducts in-services and direct face-to-face sales calls, as well as represents the facility in community-related activities and marketing events.
