The role focuses on person-centered engagement, helping individuals pursue interests, volunteerism, and community participation through scheduled activities and supports. Responsibilities include coaching individuals to achieve goals, following behavioral plans, and maintaining accurate progress documentation in a Day Hab setting.

Responsibilities

• Collaborate with individuals to identify interests, strengths, and goals and develop daily/weekly schedules.
• Provide coaching to help individuals complete tasks and reach goals.
• Follow behavioral care plans and promote positive behaviors.
• Document progress and outcomes for continuity of care.
• Assist in safely transporting individuals as needed and ensure accessibility.
