Job summary

Overview

We’re hiring a Community Integration Specialist to support individuals receiving services to become more active, confident, and connected within their community.

What You’ll Do

Support individuals in the community as they attend classes , participate in events, voluntee r , and enjoy their hobbies.

Help people build independent living skills including planning, communication, and basic money management.

Encourage friendships, social engagement, and stronger community connections.

Provide safe, reliable transportation to community outings and activities.

Document progress through the ir Individualized Service Plan (ISP).

Work closely with families and disability service stakeholders to support each person’s goals.
