CRT Community Integration Specialist
Community Rehabilitation and Treatment Services (CRT)

ABOUT NKHS

NKHS is a Designated Agency contracted with the State of Vermont to provide the highest quality of service in the areas of adults with intellectual/developmental disabilities, children and youth with serious emotional disabilities, and adults with mental health and substance use challenges. We serve the populations of Caledonia, Essex, and Orleans counties through a wide variety of targeted programs for individuals of all ages. NKHS has over 450 employees providing case management, community and home supports, residential care, psychiatry, medication management, therapy, vocational supports, school based counseling, emergency care, and respite services. These services are provided annually to nearly 4,000 of our local community members, helping them overcome challenges and achieve health in mind, body, and spirit. Visit us at NKHS.org.

Key Responsibilities:
  1. Facilitate Individual Plan of Care (IPC) Development: Collaborate with a dedicated team to actively participate in crafting personalized Individual Plans of Care. Your input will contribute to creating a roadmap for individual success and well-being.
  2. Support in Community Settings: Embark on a fulfilling journey as you support assigned individuals in diverse community settings. Your role is pivotal in fostering an inclusive environment where everyone can thrive.
  3. Assess Individual Progress: Provide insightful written assessments of individual progress as outlined in the IPC. Your keen observations will help tailor support strategies to meet individual needs effectively.
  4. Provide Coaching and Supervision: Offer personalized coaching and supervision to individuals in community settings, promoting skill development and personal growth.
  5. Skills Training in Daily Living Activities: Deliver comprehensive skills training in all activities of daily living, enhancing individuals' abilities and fostering independence.
  6. Relational Support: Assist individuals in relating to family and community in a competent and respectful manner, fostering meaningful connections and social integration.
QUALIFICATIONS
  • High school diploma or equivalent.
  • Experience working with individuals who are struggling with severe mental illness challenges.
  • Valid driver's license and appropriate vehicle insurance required.
SALARY & BENEFITS
  • Competitive Salary $18.00/hr plus, depending upon experience
  • Monday - Friday work week.
