Job summary

ServiceSource Inc in Utah seeks a dedicated support professional to assist adults with disabilities in our PARC Community Integration Services program. You will help monitor safety, support daily health and wellness activities, and promote independence through structured activities.

This role requires patience, strong communication, and the ability to document progress daily; responsibilities include personal care assistance, behavior support, transportation, and collaboration with the Program

Qualifications

• High school diploma or GED required with experience supporting adults with disabilities preferred.
• Strong verbal and written communication skills and ability to follow program guidelines.
• Ability to work as part of a team and engage participants in activities.

Responsibilities

• Provide support and training to groups of participants to ensure safety and well‑being.
• Assist with personal care, mobility, behavior support, and medication self‑administration guidance.
• Document progress daily, including evaluations, support plans and incident reports.
• Facilitate activities to promote independence, inclusion and community participation.
• Transit participants to events and monitor meals, breaks and recreation areas for safety.
