Responsibilities

• Provide active engagement and support for individuals in community integration, training, leisure, communication, mobility, behavior, and personal care.
• Implement Person Centered Plans outcomes and document PCP progress.
• Maintain a safe, clean, and orderly environment; report unsafe conditions and medical concerns.
• Keep accurate PCP documentation and report missing or inaccurate information to the Program Manager.
• Communicate with families, guardians, residential providers and Support Coordinators about illness, incidents, and progress.
• Perform related duties as assigned.
