Job summary

Qualifications

• High school diploma or GED required.
• Must pass Virginia DBHDS fingerprinting and background check.
• Must pass ODD assessment and competencies.
• Drivers must have a valid license and may need online driving training.

Responsibilities

• Provide active engagement and support for all individuals, including community integration, training, leisure, communication, mobility, behavior, personal care, and safety.
• Develop and document Person Centered Plans (PCP) and maintain progress notes.
• Maintain safe, clean, and orderly environment and report unsafe conditions or medical concerns.
• Maintain accurate records per PCP and regulatory requirements.
• Communicate with families and coordinators about participant status and progress.
