Keystone Human Services seeks Community Participation Associates to support adults in achieving employment potential and greater independence through person-centered planning and meaningful community participation. You will develop supports, explore options, complete discovery, and help individuals connect with employers, while coordinating transportation and teaching natural supports.

Qualifications

• Basic computer skills with ability to learn new programs and systems.
• Strong verbal and written communication skills.
• Ability to complete trainings including Medication Administration, First Aid, and CPR.
• Valid driver's license with daily access to a private vehicle.
• Successful completion of pre-employment checks and background screenings.

Responsibilities

• Develop appropriate supports for individuals based on their strengths, needs, and interests.
• Explore potential community participation options using person-centered tools.
• Complete the discovery process to identify employment themes and ideal conditions.
• Develop potential employment ideas for individuals.
• Target individualized goals to negotiate with potential employers.
• Teach and model natural supports to aid in success and coordination of transportation.
