Keystone Human Services is seeking Community Participation Associates to help adults increase their independence and meaningful community participation. The role focuses on developing supports, exploring community options, and coordinating transportation as part of a person-centered plan in Whitehall, PA.

The position requires a valid driver's license, ability to complete trainings (including CPR/First Aid), and strong communication.

Qualifications

• At least 18 years of age.
• Valid Driver's License with daily access to a privately maintained and insured vehicle.
• Ability to successfully complete required trainings including Medication Administration, First Aid, and CPR.
• Ability to complete trainings including Medication Administration, First Aid, and CPR.
• Valid driver’s license with daily access to a privately maintained and insured vehicle.
• Background checks required (MV and criminal).

Responsabilités

• Develop supports for individuals based on strengths, needs, and interests.
• Explore community participation options using person-centered tools.
• Complete discovery to identify ideal employment conditions.
• Develop potential employment ideas and negotiate with employers.
• Coordinate transportation as needed.
• Teach and model natural supports to aid in individuals' success.
• Assist with coordination of transportation.
