The Community Based Case Manager will develop and support programming to help clients achieve goals, build independent living skills, and navigate the community with planned supports.

Qualifications

• High school diploma or GED.
• Valid driver’s license and acceptable driving record.
• Current First Aid, CPR and Crisis Intervention certificates or be able to obtain these within thirty (30) days of hire.
• Pass criminal background check (CORI) and National Fingerprint Background Check.

Responsibilities

• Provide case management and skills training support to clients in areas such as career exploration, hard and soft employment skills, college campus inclusion, communication, decision-making, social skills, transportation goals, community safety and activities of daily living.
