Job summary

You’re With Us! is seeking a Community Based Case Manager in Worcester, MA to support clients with disabilities to develop independent living skills and participate in community activities.

You will provide case management, skills training, and transportation, while documenting progress and collaborating with families and service providers.

Requires HS diploma, valid driver's license, and ability to obtain first aid/cpr; background checks required.

Qualifications

• High school diploma or GED required.
• Must have valid driver’s license and clean driving record.
• Current First Aid, CPR and Crisis Intervention certificates or be able to obtain these within thirty (30) days of hire.
• Pass criminal background check(CORI) and National Fingerprint Background Check
• Associates Degree in Human Services or related field, or 1+ years of experience in day/residential program for individuals with disabilities.
• Direct Care experience working with individuals with Intellectual/Developmental Disabilities.
• Excellent organizational, communication, and writing skills.
• Demonstrated ability to work independently and with a culturally diverse population.
• Ability to write clear, concise and accurate documentation.

Responsibilities

• Provide case management and skills training support to clients in areas such as career exploration, hard and soft employment skills, college campus inclusion, communication, decision-making, social skills, transportation goals, community safety and activities of daily living.
• Develop programming and curriculum on a case by case program that allows opportunities for clients to develop skills to achieve their goals in a safe and controlled environment
• Review case records and programming and curriculum on a regular basis to ensure compliance with You’re With Us! Standards.
• Provide transportation to participants in the candidate's own vehicle as needed.
• Participate in implementing and monitoring curriculum and treatment plans, completing all necessary programmatic documentation and reporting significant concerns to You’re With Us! in a timely manner.
• Develop and run group activities in the community.
• Act as an advocate for You’re With Us! clients and their families.
• Develop and maintain positive relationships with guardians, family members and other service providers.
• Perform other related work duties as assigned by senior You’re With Us! staff.
