Job summary

Community Medical Services in Phoenix, AZ, is seeking a Community Impact Specialist to foster relationships with community partners and coordinate care for individuals in recovery. Qualifications

• Experience in substance abuse rehabilitation and/or opioid treatment programs (preferred).
• Bilingual (preferred).
• Ability to travel within the region, including to all CMS locations as needed.
• Culturally competent and sensitive to individualized client treatment needs.
• Valid Driver License and Motor Vehicle Clearance (satisfactory driving record for 39 months).

Responsibilities

• Identify and access community resources.
• Provide timely and accurate documentation of client/agency encounters and coordination activities.
• Maintain outreach program data in databases such as QuickBase, Methasoft, and CRM.
• Coordinate care with CMS clinics and system partners and provide requested documentation.
• Support special populations and projects with intake documentation and outreach.
• Attend meetings to advocate, educate, and strengthen partnerships.
