Job summary

Mercy Housing & Shelter Corp seeks a Community Social Worker I to help clients achieve independence and housing stability through comprehensive support and advocacy within Hartford, CT.

You will enter case notes in GIRP within 72 hours, develop individual plans, coordinate housing searches with the Housing Specialist, provide crisis intervention, and maintain HMIS data accuracy while connecting clients to health and social services.

Qualifications

• Bachelor’s degree or equivalent experience required.
• Experience with homeless populations and mental health/substance use preferred.
• Strong documentation and case-management skills.

Responsibilities

• Enter client case notes within 72 hours using GIRP format and complete assessments.
• Provide planning and active support for housing, mental health, and services.
• Collaborate with Housing Specialist on housing searches, recertifications, and inspections.
• Advocate for clients with landlords and partner agencies.
• Offer crisis intervention to support client stability.
• Maintain HMIS/Smartsheet data input and integrity.
• Attend CAN meetings and participate in case management trainings.
• Travel and network in the community to access resources.
• Coordinate transportation for medical appointments.
